Eligibility Criteria
Hospice eligibility is best determined by scheduling a complimentary hospice consultation. Hospice care eligibility is based on a person's medical condition and life expectancy. Common factors considered for hospice care eligibility include a prognosis of six months or less if the illness follows its expected course, a decline in functional status, and a need for skilled care to manage symptoms and enhance quality of life.
Instructions for Referral

You do not need to be a physician to make a hospice referral. Anyone can refer a friend or family member to St. Croix Hospice for a complimentary hospice consultation. Visit www.stcroixhospice.com/refer to complete a referral form, and someone will contact you within 24 hours. You can also submit a referral over the phone by calling (855) 278-2764.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
