Upper Des Moines Opportunity Outreach Clay County will be offering Easter baskets to clients. New clients must complete an intake before March 22nd, 2024, be income eligible, and live in Clay County. Please call (712) 262-7409 to reserve your basket (baskets must be picked up on March 22nd during your assigned time slot).
As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.
Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
