Community Health Center of Spencer

Community Health Center of Spencer offers medical, dental, and behavioral health services for patients of all ages and in multiple languages. We serve all people, whether payment for services would be made under Medicare, Medicaid, or CHIP; or the individual’s race, color, sex, national origin, disability, religion or age. No one will be denied access to services due to inability to pay, our discounted/sliding fee schedule is available based on family size and income.

Ike & Ivy Learning

We are an autism center that provides support and services for children with an autism spectrum disorder diagnosis or related diagnosis. We offer Applied Behavior Analysis (ABA) Therapy. We also provide trainings, education and CEUs for organizations in the community.

St. Croix Hospice is a nationally renowned, award-winning hospice agency that is committed to round-the-clock availability, prompt response and same-day admissions–including during evenings, weekends and holidays. Set apart by specialty care programs and bereavement services, St. Croix Hospice has a compassionate team that provides physical, emotional and spiritual support to patients and loved ones 24 hours a day, 365 days a year and wherever a patient calls home.

Birthright

Birthright understands the challenges related to unplanned pregnancies. Birthright is available to women for as long as they need them. They offer love, friendship, and support to women who are pregnant or think they may be pregnant. Birthright is interdenominational and is not involved in any political activities, or lobbying. Birthright’s focus is on loving the mother, reminding her that there is hope and ensuring she is not alone.

Elderbridge Agency on Aging–Health, Prevention, & Wellness

Health, prevention and wellness programs are critical to ensuring that older Iowans are informed about and have access to services and evidence-based activities that can significantly improve their health, function and quality of life and reduce the need for more costly medical interventions. These programs include congregate meals, nutrition counseling and education, and healthy movement.

Clay County Behavioral Health LINC (Local Initiative for Networking and Collaboration) is dedicated to connecting providers and organizations through partnership and education to help improve access and effectiveness of behavioral health and addiction recovery services in Clay County, Iowa. Our mission is nurturing hope, health, and wellness through community collaboration.
Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
