Elderbridge Agency on Aging–Health, Prevention, & Wellness

Health, prevention and wellness programs are critical to ensuring that older Iowans are informed about and have access to services and evidence-based activities that can significantly improve their health, function and quality of life and reduce the need for more costly medical interventions. These programs include congregate meals, nutrition counseling and education, and healthy movement.

Best Care for Better Babies & Children–Clay County Public Health

Best Care for Better Babies and Children is a family support home visitation program serving pregnant women and parents with children from 0-5 years of age.

Clay County Public Health

Clay County Public Health works in partnership with agencies in Clay County and the Iowa Department of Health & Human Services to promote and protect the health of Clay County residents, with a focus on wellness and prevention.

Clay County Behavioral Health LINC (Local Initiative for Networking and Collaboration) is dedicated to connecting providers and organizations through partnership and education to help improve access and effectiveness of behavioral health and addiction recovery services in Clay County, Iowa. Our mission is nurturing hope, health, and wellness through community collaboration.
Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
