Hospice care is provided at home, or assisted living, long-term care or hospice facility. Hospice care focuses on comfort, dignity and peace for you and your family. A team of physicians, nurses, social worker, spiritual counselors, hospice aides, pharmacists, and volunteers provide equipment and medication support, pain and symptom management, family and caregiver support, 24-hour on-call nurse, and financial guidance. We also offer music and massage therapy.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
