The Immunization Clinic provided by Clay County Public Health Services of Spencer Hospital serves families with infants through age 18.

Eligibility Criteria
We vaccinate all children regardless of healthcare coverage. No one is turned away for inability to pay.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
