Community Housing Access Resource Team (CHART)

All Hours

Providing compassionate and coordinated response to prevent the recurrences of homelessness. This team of navigators will help assist those who are homeless find resources to secure basic needs and housing. This service is accessible to providers only.

Eligibility Criteria
18 years or older, have intention to sign a 1-year lease within 30-mile radius of Clay County and must be able to verify two or more of the following: 1) Recent verifiable physical address in Clay County; 2) Current employment within Clay County; 3) Children’s current school attendance in Clay County; 4) Have an established service provider in Clay County; 5) Individuals within Clay County who provide a positive support system. Ineligible if transient or have no ties to Clay County. If ineligible, CHART is able to provide transportation to the nearest homeless shelter or a family/friend’s home, whichever is closest.

As a reminder, the 24 hour Crisis Line & Mobile Response is also available for a mental health emergency at 1-844-345-4569. Mobile Crisis is also able to help assess the need to involve a CHART Navigator.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
