The Medicare Savings Program includes four separate programs. LIS is administered by the Social Security Administration. This program helps lower Medicare Part D prescription drug costs including premiums, deductibles, and the costs of drugs on the plan’s formulary for individuals who meet the program’s income and resource eligibility requirements. MSPs are administered by state Medicaid agencies. The program has four levels of benefits: Qualified Medicare Beneficiary (QMB), Specified Low-Income Medicare Beneficiary (SLMB), Qualifying Individual (QI), and Qualified Working Disabled Individual (QDWI). MSPs help pay the Medicare Part B premium for beneficiaries who meet the program’s eligibility requirements. If eligible for QMB, beneficiaries receive assistance to pay their Medicare Parts A/B deductibles, co-pays, and co-insurance.

Eligibility Criteria
Elderbridge serves individuals 60 years of age and older. They serve those under 60 with disabilities. Elderbridge also serves caregivers.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
