The Family Caregiver Program works to provide family caregivers with the resources they need to enhance and extend their ability to care for a loved one. Caregiver Support Groups provide you with an opportunity to connect with other family caregivers. Training & Education provides information that will assist you in your caregiving role. Respite Care offers temporary, substitute supports for your care recipient in order to provide you with a break. There are a few key programs: 1) Information & Assistance--Information about services available to help caregivers in their role (e.g., chore services, home-delivered meals, adult day care), assistance in identifying service needs, and referrals to local service providers; 2) Options counseling/personal assistance to help caregivers sort through challenges and identify possible solutions, organization of peer support groups, caregiver training to assist caregivers in the areas of health, nutrition, and financial literacy, and in making decisions and solving problems relating to caregiving roles; 3) Information about respite care services to temporarily relieve caregivers from their caregiving responsibilities and assistance in finding resources to pay for respite care; 4) The Older Relative Caregivers program is intended to assist grandparents or older relative caregivers, who are 55 years or older and are caring for a child 18 years or younger. This program will provide information to grandparents about available services and assist them in gaining access to those services.

Eligibility Criteria
Elderbridge serves individuals 60 years of age and older. They serve those under 60 with disabilities. Elderbridge also serves caregivers.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
