Grand Avenue Free Medical Clinic provides healthcare services, primarily monitoring and treating chronic conditions. We perform lab tests and routine school or athletic physicals.

Eligibility Criteria
Non-insured or under-insured patients are welcome to come to the clinic.
Instructions for Referral

There is no available phone. Patients can be instructed to come to the clinic on the third Thursday of the month. They can use the north entrance of the First Baptist Church. Registration is between 4 – 6 P.M. Patients are seen on a first-come, first-served basis.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
