Seasons Center for Behavioral Health–Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is designed to assist individuals who have not been successful in other outpatient programs. This program provides wrap around services that meet the individual in the community to keep them as independent as possible. Services include: Mental Health Therapy, Substance abuse counseling, Registered Nurse, Peer support and medication management.

Please fill out and send in the referral form found on the ACT web page: https://seasonscenter.org/assertive-community-treatment.php and any information you have to help. Identify what the individual has tried and been unsuccessful with, and additionally, any treatment, hospitalizations or medications.
