Iowa’s 1st Five Healthy Mental Development Initiative is a public-private partnership between the Iowa Department of Health & Human Services (HHS) and primary care providers in 88 Iowa counties, including Clay County. By partnering with primary care providers and providing training on universal screening and referral assistance, 1st Five supports the healthy cognitive, social, and emotional development of Iowa children from birth through five years.

Eligibility Criteria
1st Five is a free, voluntary initiative for all families with children birth to 5 years of age. There are no income limits or insurance requirements to receive the benefits of 1st Five. Referals are made by primary care providers.
Instructions for Referral

1. The child (age birth to first day of 5th birthday) attends a well-check at their medical home of choice.
2. The primary care provider utilizes a universal screening tool (ASQ-3, ASQ-SE, SWYC, MCHAT) to identify any concerns about the healthy mental health development of the child.
3. The provider identifies the patient’s needs and completes a referral by faxing to 1st Five.
3. 1st Five contact the family and link them with appropriate intervention services.
4. 1st Five Developmental Support Specialists follow up with the provider on the patient’s status.

Important Notice: Care Connections of Northern Iowa Will Close Operations on June 30, 2025

As part of Iowa’s statewide reorganization of behavioral health and disability services, Care Connections of Northern Iowa (CCNIA) will cease all operations effective June 30, 2025. Clients will be transitioned to new service coordination under the awarded Administrative Services Organization and Disability Access Points.

Providers are requested to submit all final invoices for services rendered prior to July 1, 2025, no later than August 10, 2025.
